Breast Implant Position – Over or Under the Pectoralis Muscle? Which is right for you?

“The placement of the breast implant is one of the key decisions to discuss with your plastic surgeon. While most patients in my practice have the implant placed under the muscle, there are cases where over the muscle may be beneficial,” says Dr. Paul Wigoda.

Take a moment to look at the diagram below to see the size and location of the muscles that make up your chest wall. You will notice that the pectoralis major muscle is a large fan-shaped muscle that does not cover the lower, outer aspect of your chest wall. Generally in breast augmentation surgery the pectoralis major muscle will be detached from its lower origin along the ribs and the implant will be placed under it leaving only about half the implant covered by the muscle. The bigger the implant, the less of it that is covered by the muscle.

 

The pectoralis minor muscle is much smaller and lies under the outer edge of the pectoralis major. The serratus muscle lies along the sides of your chest wall. The main sensory nerves to your breast come up between the slips of this muscle to enter the back of your breast tissue and from there to the rest of your breast. Generally surgeons place the implants on top of the pectoralis minor and serratus muscles, but below the pectoralis major muscles.

There are pro’s and con’s to having your implants placed over or below the muscle. Here are some things to consider:

Over the Muscle (Subglandular) Breast Implant Placement:

  • Pros: When textured implants are used, the capsular contracture rate is only 5–10%. If you have a moderate amount of breast tissue (a B cup or larger), this can be an excellent choice and produce very natural-looking breasts that age very well. Also, recovery is usually easier than when the implants are placed in a submuscular position.

  • Cons: The use of smooth implants in a subglandular position have an increased rate of capsular contracture compared to other options. Additionally, when textured implants are used in thinner women with small breasts rippling may be visible in the upper and inner areas. It is also easier to see the upper edges of the implants when they are placed over the muscle.

Under the Muscle (Subpectoral) Breast Implant Placement:

  • Pros: Using smooth implants under the pectoralis muscles reduces the capsular contracture rate to less than 10%. In thinner women, the added bulk of the muscle over the upper and inner aspects of the implants will make the edges less visible and the curve of the breast more natural. When textured implants are used, the rippling in the areas under the muscle is usually invisible.

  • Cons: The recovery is typically thought to be more uncomfortable. However, with Dr. Wigoda’s painless breast implant procedure, patients most often have implants placed below the muscle and have little or no pain. It can take several months to see your end results because the implant tends to start out high before it settles into its final position, lower on the chest. When the pectoralis muscle is contracted during exercise or in some daily movements, the muscle may flatten the implants and push them outward. This can change the shape of the breast. However, this does not happen in all patients. Occasionally, an indentation over the implant can be seen along the edge of the muscle but again, this is not typical. When there is a moderate amount of breast tissue that is lax or becomes lax in time, it may sag below the position of the implant that is held in place by the muscle.

You would never guess I had breast augmentation surgery one month ago if you looked at my schedule. I’ve been to Jupiter, Fort Lauderdale, Orlando, Virginia Beach, North Carolina’s Outer Banks, Chicago and San Francisco. Two concerts, a 4-day business conference and I started running on the treadmill last week.

My scars are barely noticeable and I feel like my new additions have always been part of my body. My implants are still sitting a little high on my chest, but they have dropped a little each day, so I think I am on track for the two to three months for overall results.

To Massage or not to Massage after Breast Implants?
When I run into girls who have breast implants and we share the common bond, everyone tells me to massage my breasts for best breast results. So that’s what I have been doing, and it helps!

I’ve been drinking lots of water and exercising more too.

I wore a bikini top for the first time this weekend, which was exciting and invigorating. Confidence booster 100%.

What do you tell the kids about breast implants?
I was nervous what my eight-year-old daughter would think or say. But it was actually pretty easy. She did notice, but when she asked “why are mommy’s boobs bigger and what are those little cuts,” I told her it was like when girls get a different hair color or hair cut – sometimes, they might want a different boob size, bigger or smaller. She said, “Ohhhhhh!”
That was easy…and true!

Check in to Dr. Wigoda’s on @foursquare, of course! A pregnancy test confirms I am NOT pregnant, thank you very much. In the prep room the Wigoda plastic surgery A team gets me dressed for surgery in style, IV in, and whatever they are telling me is not registering because I can’t think straight. With Navah and Don as my support team, how could I go wrong? Navah captured the YouTube moment while Don filled my meds.

Dr. Wigoda comes in to tell me I need to make a decision on size. I’m nervous to be too big, but decide on 370 cc (the bigger of my two choices).  According to everyone at The Aesthetics Institute, 90% of breast implant patients wish they went bigger, the other 10% are ok with size, nobody wishes for smaller. So why be the first? I go with bigger.

Surgery table in sight, trying to get those last text messages in on iPhone, kisses to my boyfriend and Navah. Will I wake up to see them again? (random thought running through my head) So if I die, at my funeral the whisper would be: How did she die? Answer: On the operating room table getting breast implant surgery. This is so anti-me, I decide God would not let that happen.

The Wigoda “A” team ask me what kind of cocktails I like, I say cosmo, they start the IV and I’m out. Anesthesiologist Dr. Paul Rodriquez makes the perfect cocktail!

I hear someone softly saying “Lisa.”  I open my eyes and feel such a sense of relief that I am alive, I look down at my chest, I think ‘ok, nothing crazy’, looks good. I feel emotional and want to hold everyone’s hand. Nurses Erica, Rocky, Lucy could not have been more comforting, like my new BFFs! By 11:45 I was back in my Juicy tracksuit and in the car on my way home (Don in the driver seat).

I am in no pain, just feeling groggy from the anesthesia. I eat a Jello, some chicken broth and take my prescribed pills of pain killers, steroids, muscle relaxers and antibiotics. I feel like I can talk on the phone and text, but Don says I shouldn’t because I won’t remember anything. I am super thirsty and drink tons of water. Dozing in and out of Oprah, Rachel Ray and Dr. Oz seems like a nice break from social media networks.

4:00 pm

I ate some chicken and salad and have been walking around a little. I can even walk up and down the stairs and get out of bed myself! Dr. Wigoda just called to check in with me. He said keep lifting my arms every hour, I’ll be following doctor’s orders!

Nurse Don’s (my boyfriend) shift ends, he feels so good about me feeling good; he is off to the gym and I am off to Twitter!

Breast Augmentation Scars: Incision Options

One of the first questions many women ask when considering breast augmentation is: Where will the incision go and how will the scar look?

It’s understandable that women want to look good, and in most cases, breast augmentation is for cosmetic purposes, so the concern about breast implant scarring is normal.

Dr. Paul Wigoda, a Fort Lauderdale breast augmentation plastic surgeon gives his insight to the types of breast implant incisions and the pros/cons of each.

Breast Augmentation Incisions and Scars
There are three different breast implant incisions that are offered for breast augmentation, including:

  1. Inframammary: Below the breast in the fold
  2. Periareolar: Around the lower half of the areola (nipple)
  3. Transaxillary: In the armpit

According to Dr. Wigoda, some patients may be better candidates for using one incision over the other but most patients are good candidates for at least two if not all three types of breast augmentation incision. In most cases, placement of incision is up to the patient and where they would prefer to have their scar. The good news is, that in the vast majority of Fort Lauderdale breast augmentation cases the scars heal well and eventually are minimally noticeable.

Read on for the pros and cons of the three different types of breast augmentation incisions and scaring.

Breast Implant Incision: Below the breast in the fold (Inframammary)
Pro: This is the most common incision used by plastic surgeons. The scar from this incision is not visible when sitting up or standing. Also, in the future, if a woman’s breast implants need to be removed, most surgeons will do so through this incision. If the woman’s original implants were placed through a different incision, they will now have scars in two locations.
Con: The incision can be seen when the patent is laying down but in most cases the scar is unnoticeable after six months.

Breast Implant Incision: Around the lower half of the areola (Periareolar)
Pro: If placed correctly, the periareolar scar heals very nicely and is often difficult to find even by a trained eye.
Con: In order to use this incision the areola has to be larger than 3.4 to 4.0 centimeters across.

Breast Implant Incision: In the armpit (Transaxillary)
Pro: With the introduction of endoscopic surgery, breast augmentation through the armpit has become more common.
Con: For the first six months after breast augmentation the scar will be noticeable when a woman raises her arms and is wearing a sleeveless shirt. Women considering Fort Lauderdale breast augmentation and live in sunny South Florida may want to take this con into consideration.

Fort Lauderdale Breast Augmentation Diaries Follow-up

It’s been 17 days since I had my Fort Lauderdale breast augmentation surgery with Dr. Wigoda and I wanted to share more about my experience and follow-up appointment.

The purpose of this appointment was to check the healing of my breast augmentation and give me the chance to talk to Dr. Wigoda about any follow-up questions or concerns. I have already had my stitches out and have been doing great.  I am happy, excited, and am doing normal activities including going to beach and following my daily routine around the house.

At the beginning of my appointment Dr. Wigoda checked my breasts and reported that the tops of my breasts (the swollen part) started dropping and that my breasts were becoming fuller at the bottom. Dr. Wigoda also showed me my breast augmentation scars for the first time and I was very surprised and happy because you can barely see them. He did a great job!

Dr. Wigoda also instructed me to start massaging my breasts to help move the dropping process along. After today I am able to start working out again in moderation, like bicycling at the gym or swimming, but I have been instructed not to lift weights to do anything to strain my chest.

I am very happy to have heard from Dr. Wigoda that everything is healing great. My next visit will be in three weeks.

Hello everyone. My name is Karoline; I am a 27-year-old woman who has had breast augmentation done at Dr. Wigoda’s Aesthetics Institute in Fort Lauderdale, FL and I am very pleased to share my story with you. Please check out my before picture and after video and let me know what you think.

I grew up in a family where most of the women had small breasts so until recently; the size of my chest was not really an issue for me.  However, I have always admired those with voluptuous breasts and dreamed of having nice cleavage in the low-cut dress without padded bras. I realized that the only way to make my dream come true was with breast augmentation.

Before making any decisions on to have my breast augmentation in Fort Lauderdale, I researched the Internet and read Web sites, articles, blogs, and even found some useful information on YouTube. Upon searching for Miami/Fort Lauderdale breast augmentation, I came across the name Dr. Wigoda and I was eager to look into his Web site. It was very important to me to read out about Dr. Wigoda’s education and training where, in addition, I found out that he has been awarded for his great work and also made media appearances. I was very impressed and called the office immediately to set up my free breast augmentation consultation.

Visiting Dr. Wigoda’s Aesthetics Institute was a pleasant experience. It is very clean, professional and everyone was very nice. Dr. Wigoda is very informative and addressed all my questions and concerns about breast augmentation very clearly. After this time spent with Dr. Wigoda, it was a simple decision to pick him as my surgeon for breast augmentation.

I made my breast augmentation appointment that same day and one week later I was back at the Fort Lauderdale Aesthetics Institute for my surgery.

The morning of my surgery was a little nerve racking but the staff and Dr. Wigoda was very comforting and caring. They prepared everything for my surgery and soon enough the anesthesia was kicking in and I fell asleep. The surgery went very smooth, and as I woke up, I was surprised that everything went so fast. My chest was a little sore and my breasts felt tight but I was feeling great!

Soon enough my husband took me home and I rested in bed for the rest of the day. That first night, I was a little uncomfortable because I had to sleep on my back, which I don’t really like – but I made it through and was able to get a good night sleep. On each one of the next two days I got a call from Dr. Wigoda who was checking up on my recovery. He also offered me his cell phone to contact him throughout the weekend with any concerns or questions.

The first week after my surgery was a very pleasant experience. I felt great. I was taking my medication and working through the rehabilitation series, which included lifting my arms every hour and avoiding lifting heavy objects so I would not strain my chest. I also have to add that I didn’t experience any serious pain – I only took four pain pills because of soreness and slight discomfort.  All in all, I felt great and got my stitches out after just a week.

Two weeks have already past since my surgery and I feel amazing. My breasts are still swollen but I couldn’t wait to wear my new bathing suit, so I went to the beach! For the next couple of weeks I’ll be seeing Dr. Wigoda once a week for a check up and I can’t wait for my final results.

Plastic Surgery Experience with Miami Breast Augmentation

So today is my last update on my breast augmentation surgery with Miami plastic surgeon, Dr. Wigoda.  I just wanted to reiterate how happy I am with Dr. Wigoda and his staff.  My breast augmentation surgery went great and I couldn’t be more pleased with my results.  You will see my before and after pictures below.  I went from an A-cup to a C-cup when I had my breast implant surgery last month and I feel 100% on my game!
