Do breast implants need to be replaced every 10 years?

That’s one question Board Certified Plastic Surgeon Dr. Paul Wigoda frequently hears from his breast augmentation patients.

“That’s a common myth about breast implants that I hear all the time from patients, but it’s not true,” said Dr. Wigoda in one of his plastic surgery Facebook Live Q&A sessions. “Breast implants do not have to be changed every 10 years.”

Dr. Wigoda goes on to say that he has had patients who had waited for a very long time to get implants, because they don’t want to replace them just a few years down the road and don’t want to undergo surgery regularly. Other patients come back after 10 years because they think they need to get them replaced. In actuality, this is just one of the many myths about breast implants.

 

So, do breast implants ever need to be replaced?

Yes. There are times when breast implants need to be replaced, just not on a regular basis.

Implants don’t need to be changed or removed unless there is a problem or the patient wants to make a change with elective surgery such as  wanting to go bigger (or less commonly, smaller), or wanting a lift and deciding to change them even though they are intact.

“If implants rupture or deflate you would replace them. As long as they’re not ruptured, hard, or have capsular contracture and you don’t want a breast lift, they don’t need to be replaced,” said Dr. Wigoda. “Even if they’ve been in for 10 to 15 years you do not have to change them. Implants may not need to be changed for 20, 30, or even 40 years.”

Breast augmentation procedures are much different than they were 20 years ago. The shells are much stronger and it’s very likely silicone implants will last 20 to 30 years. Only one out 20 women will have a ruptured implant after 10 years and we have yet to receive data for ruptured rates at longer time periods.

How do I know when my breast implants need to be changed?

With saline breast implants, you will know one ruptured because it deflates pretty quickly. It may take a week or two but you will definitely know because of the physical difference. The saline solution can be absorbed by your body without posing any health risks, but you will want to either remove both shells or replace the damaged one to get back to a symmetrical appearance.

Silicone breast implant ruptures may not be noticeable. In fact, you may not know about it all but it’s not dangerous and the silicone typically remains trapped within the implant shell. If it does escape, the silicone does not spread to your breast tissue or body and you will not get sick. If you are in an accident, your plastic surgeon may suggest you get an MRI, just so you know what you’re dealing with.

So don’t hesitate if you really want breast implants because you think you will need plastic surgery every 10 years. While ruptured breast implants may impact how long your implants last, they are
not highly likely and shouldn’t keep you from a plastic surgery procedure that can increase your confidence and self-esteem.

The Healing Process Begins – One Day After Breast Implants Surgery

Just one-day post-op, this patient sang her praise about Dr. Paul Wigoda and staff.

No Pain, Yes Gain – Three Days After Breast Implants Surgery

Two days after her initial post-op appointment, our patient reports that she is completely able to raise her hands above her head with “virtually no pain” in her chest. She does, however, explain that she has upper back and lower neck soreness as a result of “adding five pounds to [her] front.”

On day three, she was able to take a nice walk around the neighborhood but continued relaxing and using a heating pad. She also began feeling soreness around her incisions but could not feel her nipples or the skin around them at that time, which Dr. Wigoda explained was normal.

Plastic surgery has been rapidly on the rise this year. With the numbers growing for acceptance of plastic surgery, there is one number that is rapidly shrinking. Every year, more and more teens and young adults are jumping on the plastic surgery bandwagon.

With an incredible 200,000 plastic surgery procedures performed on individuals between the ages of thirteen and nineteen in 2010, according to American Society of Plastic Surgeons, what age is too young to make such a drastic and life altering decision?

With young celebrities like Kylie Jenner, who admittedly received lip injections and possibly breast implants at the age of seventeen, setting an example for young girls, the importance of body image is at an all time high. The glitz and glam of getting that “ideal” curvaceous look is consuming teens and adults across the globe. This led us to ask board-certified plastic surgeon, Dr. Paul Wigoda, one of the most popular questions buzzing around in today’s tabloids, “how young is TOO young for plastic surgery?”

“Eighteen is usually the cutoff that most plastic surgeons agree on for breast augmentation. You want to make sure the patient is fully grown physically, but probably more important, is their level of maturity” states Dr. Wigoda, who will only perform his Painless Breast Implant surgery on adults eighteen years or older. Dr. Wigoda has performed the Painless Breast Implants procedure on hundreds of women from all over the world, and notes some important factors to consider about age.

The most crucial thing to understand is that no matter how old or mature mentally you may think you are, your body may not be at that same stage of maturity. Most females reach their final adult height at age 15, but may continue to grow and fill out up until age eighteen or twenty-one depending when their menstruation began. Placing implants on someone as young as seventeen when their body is not yet fully developed could lead to changes in the appearance of the breasts that perhaps was not anticipated or planned.

However, mental capacity and maturity is indeed a huge consideration with plastic surgery. No matter if you are a teen or adult, plastic surgery is a choice that needs careful planning and consideration as well as extensive education and cannot be done on impulse. Young adults who choose plastic surgery without proper knowledge often still have insecurities or negative repercussions afterward.

Dr. Wigoda stresses researching your plastic surgeon by reading real patients reviews, meeting with your plastic surgeon, and even taking a tour of their facility to make sure they are AAAASF certified. Remember, no matter your age, you are in control of your own body.

 

Is There An Exception To The Rule?

Isn’t there always an exception to the rule? “There are some procedures, like ear tucks, breast reductions, and rhinoplasty, where you can make the argument that having these as a teenager is reasonable.” Dr. Wigoda tells us in our interview.

Often times, teens and young children can experience pain or discomfort due to large busts, or extensive teasing and self-esteem issues due to physical appearances. Otoplasty for example, is used to pin back the ears and has extensively been proven to help the confidence of teens and children. Breast reductions are often done on women who are in a great deal of pain when the breasts impede on everyday activities. Should cases like these be denied surgery because they are too young?

Whatever the case, plastic surgery and age is an increasingly slippery slope. As plastic surgery becomes more popular, Dr. Wigoda says the most important thing a surgeon can do to combat issue is to have a policy, and to stick to it.

Dr Wigoda Shares Insight on @PRSJournal’s Study of The Perfect Breast Shape

Beauty is in the eye of the beholder and, following that logic, that should mean that the perfect pair of breasts is subject to debate. However, according to a new study called Population Analysis of the Perfect Breast, a whopping majority of people agree on what they believe are the most attractive proportions and you may be surprised by the results.

 

In the study, both men and women were asked to rank the attractiveness of four women with varying breast sizes. Each of the women’s breasts were morphed into four different proportions based on the percentage of the breast above and below the nipple. While size preferences varied, one preferred proportion was apparent.RealSelf Perfect Breast Shape

 

Of the four proportions which were 35:65, 45:55; 50:50, and 55:45, the 45:55 ratio stood out as the clear winner with both genders. However, while both sexes overwhelmingly agreed on the proportion, 90% of men selected it versus 82% of women which suggests that men actually prefer more natural appearing breasts than do women.

 

So what does that mean about the quest to find the perfect pair?

 

The 45:55 upper-to-lower pole ratio was actually found to be the most natural and attractive, according to the study. The keyword here being natural, a term that most would not associate with breast implants. While there are some women who come into the Aesthetics Institute who specifically want the “fake look,” most, however want a natural looking, just fuller, breast.

 

When considering breast implants, many women have the mentality that if they’re going to get implants, they need to ensure that they have visible results. However, they also worry that going too big will result in a pornstar-esque or “fake and stacked” appearance that they won’t be happy with…Quite the dilemma. While this study didn’t report what cup size was most ideal, it did mention that regardless of what size you choose, it can still look natural by coming as close to this proportion as possible.

 

Armed with this knowledge, women now have a language they can use to describe what they want their plastic surgeon to achieve. Likewise, it gives the surgeon a template to aspire to, rather than guess or assume. The thing to keep in mind is that while doing research, seek a plastic surgeon with excellent reviews AND who very clearly discusses the procedure and how he/she will meet your desired results. Feeling comfortable and getting honest feedback will ease your worries that you’ll end up with a drastic and unwanted appearance.

You have to be careful washing after any surgery and must follow your doctor’s recommendations as to how long to keep the incision area dry and how to care for it. How to Identify Ruptured Breast Implants

Identifying Ruptured Breast ImplantsA common misconception of breast implant patients is that the breast surgery is a one-and-done procedure. The truth is that, even without additional trauma to the chest, breast implants are not lifetime devices. Over time the breast implant will wear and can result in rupturing and may need to be replaced. According to Breast Implant Safety, the general experience among plastic surgeons tend to be that implants last between 10 and 20 years, but more on that later. Before your procedure, it is important to understand that breast implants are not permanent, the risk of rupture rate and that you learn how to identify ruptured breast implants.

What is a breast implant rupture?

A breast implant rupture occurs when breast implants fail due to gradual wearing or a trauma that causes leakage. Most women have no specific cause for deflation or leakage but some possible problems that may contribute to a rupture include car accidents, falling and overly vigorous manipulation..

Often a rupture is not readily noticeable and may not cause discomfort, however, if your breasts develop an unusually hard feeling or highly rounded appearance, it may be a sign of capsular contracture and you should call your surgeon. Capsular contracture is an immune system response to foreign materials in the body and can cause discomfort.

How do I identify a breast implant rupture?

Depending on the type of implant used, saline vs. silicone, you may not even notice a rupture.

Saline implants are more visible as they deflate and you will see the size and shape change, similar to a leaking water balloon. Not to worry, the saline solution can be absorbed by your body without posing any health risks, but you will want to either remove both shells or replace the damaged one to get back to a symmetrical appearance. If it has been a while since your implant procedure, you may want to use this opportunity to replace the undamaged implant as well to delay future rupture and additional surgeries and recovery time.

A silicone breast implant rupture may not be as noticeable immediately  — or ever — because the freed silicone typically remains trapped within the implant shell. Known as a silent rupture, this type of rupture is not thought to cause health problems but may eventually cause breast pain or changes in contour or shape. Even if you don’t show signs or symptoms upon discovery after a routine MRI, you can discuss your options with your surgeon. Unless there is a compelling reason not to have surgery, such as other medical issues, most surgeons will recommend removing the ruptured implant. You can then decide if you want to replace it or not have any implants.

How often should I check for breast implant ruptures?

The current recommendations for MRIs is to have one done three years after your breast surgery procedure and then every two years. Breast Implant Safety reports that a breast implant rupture within three years is unusual but not unheard of so there is a high likelihood that the recommendations will change in the future to having MRIs less often because the rupture rate is fairly small.

There are slight differences depending on which brand implants are studied, but it’s approximately a 5–7% chance of rupture by 10 years. The Sientra brand, which I have started using this past year, has a rupture rate of 0.4% at five years for their anatomic implants so, in my opinion, the risk of rupture is fairly low. While this brand of implants has not been around for 20 or 30 years, I suspect most of them will last at least that long.

While I tell my patients what the current recommendations are, many of them choose not to get the MRIs as recommended. In a few that have gotten them after having some sort of trauma, like a hard fall or car accident, their MRIs showed no rupture. However, I still highly recommend getting regular checkups or at least an MRI after trauma or if you have suspicions. Although not life-threatening, it is always good for you and your surgeon to know what is going on rather than wait until there is discomfort or you develop a contracture.
