A: I love my body but I’ve always had super small boobies, and when I started working out extensively they got even smaller….THE HORROR!!!  I’ve been debating the whole boob job thing for a few years, but I hadn’t found a doctor I felt totally comfortable with, until I met Dr.Wigoda. After my first consultation with him I was SOLD!!!  I’ve thought about this long and hard, and I’m just ready to feel as confident out of my clothes as I do in them…

A: When I  first came in I was nervous about going too big, but Dr. Wigoda took the time to show me an array of before and afters and of course let me try on all the different sizers and I realized what I thought sounded BIG really wasn’t, so long story short, Ive decided to go with 485cc’s…( I’m currently an A cup even though Victorias Secret lets me think I’m a “B”)

Q: How long have you been considering this?

A: Years….SERIOUSLY!!!

Q: If you had to describe your style, what would it be?

Equal parts street and sass and sophistication and class!  I like to incorporate a little bit of everything into my wardrobe and style. I’m all about that Fit-Life so I love my yoga pants and work-out gear, but I’m also a City girl so I love to rock a sexy but sporty look…snap-back caps, crop tops, jeans…and sometimes I trade in my denim for a more sophisticated business look, you know, high-waisted wide-leg slacks with a blazer, but ALWAYS ALWAYS ALWAYS have to have my STILETTOS!!!

Q: Who is your favorite celebrity?

A: J-Lo…but I love Sofia Vergaras Body!!

Gummy Bear Breast Implant Warnings: Is this Newest Option the Best?

Just what does Dr. Paul Wigoda think of Gummy Bear Breast Implants? When it comes to breast implant options, until recently traditional silicone and saline breast implants were the only choices for women. However, the FDA has now approved what is widely known as “gummy bear” implants.

These breast implants got their nickname because the implants are more stable than their predecessors and retain their shape even when cut in half, just like their candy namesake.  Currently three manufacturers, Mentor, Sientra, and Allergan are approved to sell the gummy bear implants, although the manufacturers do not approve of the use of the nickname.

 

Until recently, these type of high-strength silicone gel breast implant were only available to patients in clinical trials. Proponents of these new implants prefer them because they say they more closely resemble the shape of natural breasts. However, plastic surgeon Dr. Paul Wigoda does see some negatives with them.

 

The Downside of Gummy Bear Implants

 

“Gummy bear implants are a bit firmer than the other gel implants, and they require a larger incision,” Wigoda states.

 

“Also, they can rotate and cause the breast to have a non-optimal shape because they are teardrop shaped and not round.”

 

Wigoda suggests consulting with your plastic surgeon before you select your implant, to weigh the pros and cons of each implant type.  Form stable implants, as their name    implies, use a stiffer gel, which leads to additional firmness and less wrinkling of the implant. There is some evidence that there is less risk for a women to develop a capsular contracture around the breast implant.

 

Wigoda still prefers smooth gel implants.  “When placed under the muscle, they essentially become tear drop shaped due to the muscle compressing the upper aspect of the implant, while there is no problem when the implants rotate because they are round.”

 

Wigoda notes that all breast implants share risks of infection in rare cases, as well as bleeding and implant rupture.

Dr Wigoda on The Incision Decision: Choosing an Incision Location for Your Breast Implants

The location of your incision is one of the many options you will discuss with Dr. Wigoda during your preoperative consultation. Each incision location option has its pro’s and con’s, such as scarring, and there is not one location that is considered best for everyone. Here are the options:

1) Inframammary Incision

Inframammary incisions are placed underneath the breast just slightly above where the breast meets the rib cage. Many surgeons like this incision location as it allows them easy access to create an implant pocket above or under the muscle.

This direct access allows Dr. Wigoda to use very small incisions with saline implants, generally as small as 1 to 1.5 inches wide for saline implants and slightly larger incisions for silicone. The scar is typically not visible when standing up as the breast will cover it. This is the most common incision site used.

2) Transumbilical Incision or TUBA

A Transumbilical incision is a C or J shaped incision placed in the folds of your belly button. A small camera is used to negotiated the surgery and very small instruments are then inserted to create a path to the breasts. Tissue expanders are inserted to create the implant pocket. Using a TUBA incision, implants can be placed over or under the muscle.

While TUBA incisions have the advantage of there being no scarring on the breast area, there are many potential disadvantages. Only saline implants can be used if TUBA is selected as silicone implants will not fit through the very small incision. This is the least popular incision used.

3) Transaxillary Incision (Armpit)

These incisions are created in the armpits and are generally 1 to 2 inches long. A short path is then created to the breasts where the implants will be placed. While there is no scar on the breast, a scar will be visible in the armpit with the arm is raised. There is also a risk of injury to a sensory nerve that can cause numbness on the inner arm.

4) Periareolar Incisions

The Periareolar incision is created around the areola. This incision is commonly used if other cosmetic breast procedures such as breast lift or areola reduction are being performed in conjunction with breast augmentation. Contrary to popular belief the periareolar incision does not necessarily increase the risk of losing sensation in the nipple.

There is, however, potentially a greater risk of infection as bacteria is found in milk ducts and often these ducts are cut during the procedure. If you plan to breastfeed after your surgery should make sure to discuss this with Dr. Wigoda because periareolar incisions may not be right for you.

Dr. Paul Wigoda Reports Silicone Breast Implants Grow 72% in 2012

Faces are lifting, silicone breast implants are IN, skin is smoothing, Botox is booming and tummy tucking is far from flat, growing 70% in the last 12 years. The biggest bump Dr. Paul Wigoda noticed was the fact that 72% of breast implant surgeries in the United States used silicone implants; 28% used saline or sterile salt water implants.

The American Society of Plastic Surgeons has just released their 2012 annual statistics reporting the number and variety of plastic surgery procedures that were performed last year. The report, a virtual “state of the union” for plastic surgery profession, provides us with several interesting insights into trends in plastic surgery for the year.

“The fact that total U.S. cosmetic procedures were up last year is not surprising given that the economy is beginning to bounce back and these procedures are high on many people’s list of things to do with the extra income they are seeing.” says Dr. Paul Wigoda. Overall, cosmetic procedures in total were up a whopping 5%! The most popular procedures by volume were breast augmentation with 286,274 performed in 2012, followed by nose reshaping (242,684), eyelid surgery (204,015), liposuction (202,128), facelift (126,320) and tummy tuck (106,628).

Faces Lifting!

The biggest bump last year came in the form of facial rejuvenation procedures.

  • Facelifts up by 6% from 2011

  • Eyelid Surgery up 4%

  • Microdermabrasion up 8% with 973,556 performed

  • Soft Tissue Fillers up by 5% with 1,988,767 performed

  • Botox and Dysport up 8% with 6,134,631 performed

Why so much focus on the face? The ASPS suggests “one has to assume that economic recovery and consumer confidence plays an important role… Another factor is people entering or re-entering the workforce at an older age, needing to compete with younger applicants. Having our appearance look as good as our experience and resumes… is a strong motivation.”

Breast Implant Position – Over or Under the Pectoralis Muscle? Which is right for you?

“The placement of the breast implant is one of the key decisions to discuss with your plastic surgeon. While most patients in my practice have the implant placed under the muscle, there are cases where over the muscle may be beneficial,” says Dr. Paul Wigoda.

Take a moment to look at the diagram below to see the size and location of the muscles that make up your chest wall. You will notice that the pectoralis major muscle is a large fan-shaped muscle that does not cover the lower, outer aspect of your chest wall. Generally in breast augmentation surgery the pectoralis major muscle will be detached from its lower origin along the ribs and the implant will be placed under it leaving only about half the implant covered by the muscle. The bigger the implant, the less of it that is covered by the muscle.

 

The pectoralis minor muscle is much smaller and lies under the outer edge of the pectoralis major. The serratus muscle lies along the sides of your chest wall. The main sensory nerves to your breast come up between the slips of this muscle to enter the back of your breast tissue and from there to the rest of your breast. Generally surgeons place the implants on top of the pectoralis minor and serratus muscles, but below the pectoralis major muscles.

There are pro’s and con’s to having your implants placed over or below the muscle. Here are some things to consider:

Over the Muscle (Subglandular) Breast Implant Placement:

  • Pros: When textured implants are used, the capsular contracture rate is only 5–10%. If you have a moderate amount of breast tissue (a B cup or larger), this can be an excellent choice and produce very natural-looking breasts that age very well. Also, recovery is usually easier than when the implants are placed in a submuscular position.

  • Cons: The use of smooth implants in a subglandular position have an increased rate of capsular contracture compared to other options. Additionally, when textured implants are used in thinner women with small breasts rippling may be visible in the upper and inner areas. It is also easier to see the upper edges of the implants when they are placed over the muscle.

Under the Muscle (Subpectoral) Breast Implant Placement:

  • Pros: Using smooth implants under the pectoralis muscles reduces the capsular contracture rate to less than 10%. In thinner women, the added bulk of the muscle over the upper and inner aspects of the implants will make the edges less visible and the curve of the breast more natural. When textured implants are used, the rippling in the areas under the muscle is usually invisible.

  • Cons: The recovery is typically thought to be more uncomfortable. However, with Dr. Wigoda’s painless breast implant procedure, patients most often have implants placed below the muscle and have little or no pain. It can take several months to see your end results because the implant tends to start out high before it settles into its final position, lower on the chest. When the pectoralis muscle is contracted during exercise or in some daily movements, the muscle may flatten the implants and push them outward. This can change the shape of the breast. However, this does not happen in all patients. Occasionally, an indentation over the implant can be seen along the edge of the muscle but again, this is not typical. When there is a moderate amount of breast tissue that is lax or becomes lax in time, it may sag below the position of the implant that is held in place by the muscle.

Choosing Breast Implant Shapes and Textures With Dr. Paul Wigoda

Round, Teardrop, Smooth or Textured? Breast augmentation was the #1 cosmetic surgical procedure in 2012 , with almost 300,000 breast augmentations being performed last year in the United States. Round or Teardrop Shape?

Breast implants come in two shapes: round and teardrop. Which is right for you? Well, it depends. Here are some considerations to help you decide.


Teardrop Implants:

Teardrop implants also known as “anatomical” are contoured and shaped just like a natural breast. It takes a skilled and experienced plastic surgeon to ensure they are properly placed. Dr. Wigoda has extensive experience with teardrop implants. At your consultation, Dr. Wigoda will discuss your options with you, including the use of anatomical implants. Anatomical implants are always textured to minimize their movement and decrease the risk of the implant rotating.

Round Implants:

Round implants will give you that “Victoria Secret” look. They less natural looking than teardrop shaped implants but can deliver a higher impact result. Round implants are currently the most common implants as they give women the most lift, fullness, and cleavage many of us are looking for. Also, certain types of round implants will enhance the forward projection of your breasts. Keep in mind that women with large implants often report that others ‘know’ they have had breast augmentation. For most women, these days, this is not a concern. It all depends on you!

Smooth or Textured?

Breast implants are available in two surface choices: smooth and textured. Smooth breast implants feel like the outside of a balloon. Textured implants feel somewhat like sandpaper. Which is right for you? Below we will discuss some considerations to help you make your choice.

Smooth Breast Implants: Smooth breast implants are used in most breast augmentation procedures. Smooth implants move freely within the breast pocket and have thinner shells making them softer than textured implants. Smooth implants may be less likely to ripple than textured implants. Other benefits of smooth implants include lower cost and increased implant longevity.



Textured Breast Implants: Textured implants have been designed to limit the risk of capsular contracture, an implant complication involving scar tissue growing around the implant. However, this claim has not been conclusively proven. However, textured implants do create cohesion between the implant and surrounding tissue reducing the risk of the implant moving. Textured breast implants have been associated with a higher rate of leaking and breast rippling than smooth implants; however, textured implants have also been shown to decrease the risk of displacement.

The holidays are ideal for undergoing cosmetic surgery for several reasons. Many have vacation time from work and thus can afford to take a few weeks for the procedure and recovery.
