Jenny knew she wanted a nose job (otherwise known as rhinoplasty) before she was even a teenager. The 26-year-old healthcare professional from Miami decided after graduating college that she now had time to have the procedure. While she was hesitant to share details of other plastic surgery encounters she’d had previously, this time around she wants to share her story and help others who might be seeking similar procedures.

Jenny started her plastic surgery research where most people do, on Google. And in Miami, there are a lot of “plastic surgeons near me.”

She filtered them through by thoroughly looking at websites, researching if plastic surgeons were board certified and scouting for reviews of the surgeons.

She wanted to know what she was getting herself into. And she did. Pages with people sharing their stories of bruises and blood – the ugly side of plastic surgery – showed up in her searches. “It’s not a simple before and after,” she said.

And that’s what inspired Jenny to share her own process on her own Instagram feed. She even started a countdown before the rhinoplasty.

“I was hesitant about it because my nose was always my biggest insecurity…but I’m like, you know what this nose is going to be gone very soon so let’s do this,” she said about deciding to share her rhinoplasty plastic surgery journey on social media.

Jenny previously had a breast augmentation, but never discussed her procedure. “Although I never discussed it, there were some people that noticed, and they would actually ask me about it behind the scenes.”

 

Before, During, + After Rhinoplasty

This is what convinced Jenny to start talking about the good, the bad, and the ugly about her rhinoplasty. She wanted to be a source of knowledge and experience people could go to if they had questions. She wanted to give back like other Instagrammers had done for her when she was curious of the truth behind a “nose job.”

The Before

Dr. Wigoda kept her in the loop about everything.

While she doesn’t like needles, Jenny said she always felt safe.

“I was in good hands, I felt comfortable. Scared, but comfortable.”

And Dr. Wigoda prepped her for the after too – like keeping frozen peas around.

The After

Jenny didn’t have any pain, just discomfort. Her bruising went away quickly, and other than occasionally waking up, she was able to sleep.

“It was a rough recovery but they gave me everything I needed to make it as smooth as possible,” Jenny said about Dr. Wigoda.

Her cast came off eight days after surgery.

“Once that cast comes off on day eight, I was like here we go, like nothing happened. So it was nice, it was nice to just be able to go back to work and people weren’t like, “Hey what happened to you?” Jenny said.

Her Advice to You

Do a lot of thorough research. That’s Jenny’s number one advice.

Living in Miami, she said she hears a lot of stories of surgeries going wrong. She advises to not go to a doctor just because you hear a lot about them and instead rely on your own independent research.
She also advises making sure the plastic surgeon is board-certified and qualified.

Her last piece of advice is to seek out any bad reviews of doctors, especially if those reviews concern safety. She said she purposefully looked for bad reviews of Dr. Wigoda, but never found anything that jumped out at her.

Do breast implants need to be replaced every 10 years?

That’s one question Board Certified Plastic Surgeon Dr. Paul Wigoda frequently hears from his breast augmentation patients.

“That’s a common myth about breast implants that I hear all the time from patients, but it’s not true,” said Dr. Wigoda in one of his plastic surgery Facebook Live Q&A sessions. “Breast implants do not have to be changed every 10 years.”

Dr. Wigoda goes on to say that he has had patients who had waited for a very long time to get implants, because they don’t want to replace them just a few years down the road and don’t want to undergo surgery regularly. Other patients come back after 10 years because they think they need to get them replaced. In actuality, this is just one of the many myths about breast implants.

 

So, do breast implants ever need to be replaced?

Yes. There are times when breast implants need to be replaced, just not on a regular basis.

Implants don’t need to be changed or removed unless there is a problem or the patient wants to make a change with elective surgery such as  wanting to go bigger (or less commonly, smaller), or wanting a lift and deciding to change them even though they are intact.

“If implants rupture or deflate you would replace them. As long as they’re not ruptured, hard, or have capsular contracture and you don’t want a breast lift, they don’t need to be replaced,” said Dr. Wigoda. “Even if they’ve been in for 10 to 15 years you do not have to change them. Implants may not need to be changed for 20, 30, or even 40 years.”

Breast augmentation procedures are much different than they were 20 years ago. The shells are much stronger and it’s very likely silicone implants will last 20 to 30 years. Only one out 20 women will have a ruptured implant after 10 years and we have yet to receive data for ruptured rates at longer time periods.

How do I know when my breast implants need to be changed?

With saline breast implants, you will know one ruptured because it deflates pretty quickly. It may take a week or two but you will definitely know because of the physical difference. The saline solution can be absorbed by your body without posing any health risks, but you will want to either remove both shells or replace the damaged one to get back to a symmetrical appearance.

Silicone breast implant ruptures may not be noticeable. In fact, you may not know about it all but it’s not dangerous and the silicone typically remains trapped within the implant shell. If it does escape, the silicone does not spread to your breast tissue or body and you will not get sick. If you are in an accident, your plastic surgeon may suggest you get an MRI, just so you know what you’re dealing with.

So don’t hesitate if you really want breast implants because you think you will need plastic surgery every 10 years. While ruptured breast implants may impact how long your implants last, they are
not highly likely and shouldn’t keep you from a plastic surgery procedure that can increase your confidence and self-esteem.

TV shows like Botched! feature extreme cases that may be misleading and cause people to think “that will never happen to me.” However, just because your plastic surgeon hasn’t had a severe complication doesn’t mean you’re not at risk to experience plastic surgery gone wrong.   


Dr. Wigoda stresses the importance of researching your plastic surgeon before any procedure. Here’s what you need to do before committing to a plastic surgeon and going under the knife:

1. Make sure your plastic surgeon has hospital privileges

If you’re having a plastic surgery procedure at an outpatient clinic, make sure that, like The Aesthetics Institute in Fort Lauderdale, the clinic conducting your surgery has been inspected and accredited by the American Association for the Accreditation of Ambulatory Surgery Facilities (AAAASF).

Next, confirm where the plastic surgeon has hospital privileges. Even though your plastic surgery won’t occur in the hospital, this is a crucial step because hospitals do background checks! Hospital privileges are granted to a plastic surgeon by a committee of other doctors who check their credentials and training. They are in turn approved by the chairman of the department.  Make sure that the plastic surgeon has privileges for exactly the procedure that you want performed. For example, dermatologists may have privileges to take care of skin problems but not for liposuction or a facelift in a hospital. If your plastic surgeon doesn’t have hospital privileges, that’s a huge red flag and you need to ask why.

2. Ask for “before and after” photos of other patients

You wouldn’t buy a car before test driving it, would you? While you can’t necessarily test drive your plastic surgeon or the results, other patients’ “before and after” photos are the closest thing to it! You need to evaluate the work that has been performed to feel confident this plastic surgeon will help you reach your post-op goals. Board-certified plastic surgeons are trained to take “before and after” photographs of all surgical procedures and these should be available for patients to review during consultations. It’s understandable if your plastic surgeon cannot readily show you a photo of every one of their patients as some may opt out; however, some references showcasing the procedure in which you’re interested should be available so that you can be confident in your decision. Do not agree to a procedure with a plastic surgeon whose results make you hesitant or if you don’t feel you’ve been fully briefed and educated on all aspects of the proposed procedure.  

3. Talk to former and current patients

Again, you cannot test drive a plastic surgeon, but the next best thing is talking to someone who has already undergone the procedure you’re seeking! When applying to a new job, most people have several references and plastic surgeons are no different. If you ask for them, your plastic surgeon must be able to connect you with prior patients who can vouch, not just for their surgeon, but also for their results. Talk to former patients and ask about your surgeon’s bedside manner, availability, professionalism and anything else that will help ease your mind. Like in an interview, a person can ooze confidence and make you feel like they’re the right choice; however, if their references don’t check out, you shouldn’t check in.

4. Make sure your doctor is aware of your medical history

If your plastic surgeon doesn’t ask about your medical history after the initial consultation, you should be concerned. There are risks to surgery and your plastic surgeon should not play them down in any way. Your surgeon needs to be aware of any current or past medical problems, drugs or supplements you’re taking, etc. The plastic surgeon must know about your lifestyle and activities in order to give accurate predictions of your surgery and recovery. By asking for a detailed medical history, your plastic surgeon conveys their concern first and foremost about your safety.

Depending on your age and health, a full preoperative evaluation including past medical history is very important.  They should request lab work for blood counts, coagulation profiles, and basic chemistry profiles. Additional tests may include chest x-rays and EKGs, which are commonly performed on people over 40.  

5. Spot check! Is the office clean?

At some point during your initial visit, your plastic surgeon should give you a tour of the office so you have a chance to visualize where your procedure will occur. If you’re not provided one, ask! Scope out the surgery center and make sure that it’s up to your standards and you’re comfortable. Before you go under the knife, you need to make sure the office is clean, professional, and stocked with up-to-date equipment. If something looks off or inappropriate, be wary of it. The clinic where your surgery takes place should be as sterile as a hospital to reduce the risk of complications. As many of you avid television watchers may know, The Today Show did a week-long special on plastic surgery last week. Among the topics covered on the show was what each age bracket should know in terms of what will happen to your body, and what you can do enhance or hide it.

Age has been a trending topic this year with regards to plastic surgery. We’ve been hot on the debate about how young is too young for plastic surgery?, and now we want to know what Dr. Paul Wigoda, MD thinks about what you can do as you get older!

Dr. Wigoda, a board-certified plastic surgeon, brings years of plastic surgery on patients of all ages to the table. We asked him to set the record straight and give us some opinions on the facts being reported.  

In Your 30s:

Here are some of the tips The Today Show gives for patients in their 30s on what they can expect:

  • As you transition from 20s to 30s, prevention is KEY! It’s crucial to start thinking about what you can do to avoid developing lines, wrinkles or crow’s feet.
  • Focusing on anti-aging routines can sustain a youthful look and delay the need for surgery. Small procedures are encouraged at this age, and as always, apply sunscreen regularly to preserve the skin.
  • Lines and wrinkles become most noticeable around the eyes, mouth and forehead.
  • Just getting started? Try Botox, Dysport, Xeomin or fillers to prevent lines.

In Your 40s:

Next up, we have the 40s! This age range is when many begin to consider some sort of plastic surgery or non-invasive procedure. This is the ideal age to be very aware of what will actually be beneficial and safe, and what will just burn a hole in your pocket and lead to complications! The Today Show stresses those in their 40s to be aware of:

  • Aging becomes apparent in this stage of your life — sun damage is more pronounced and the after-effects of having children start to take a toll on your body.
  • As your face begins to lose fat, dry spots may appear as well as wrinkles that come as a result of summer days in the sun.
  • Nasolabial folds (those lines from your nose to your mouth) become deeper and alter the shape of your face — changing it from round to square. For these, injectables make all the difference. They take fat from where you don’t want it via liposuction and put it where it is deficient, according to NewBeauty. Injectable procedures can cost anywhere from $1500-$3500, take 1-3 hours and entail 5-14 days of recovery.
  • Cellulite also worsens — estrogen levels decrease and fat builds in areas naturally most affected by cellulite. Although there’s no permanent remedy for cellulite, NewBeauty recommends massage techniques, heat to stimulate collagen (Accent) and Cellulaze, a surgery performed to reduce the bands. Cellulaze is roughly $5000-$7000, takes about 1-2 hours and requires 1-2 days for recovery.

Dr. Wigoda says: “Injectables cost about $650-$950 per syringe and most patients only need one syringe. Also, the actual procedure only takes about five to ten minutes and there is virtually no recovery period.” It is very important you be aware that you are not being overcharged for procedures. Also, do the research and read customer reviews and testimonials. Dr. Wigoda prides himself on painless procedures with minimal to no pain and recovery time. If a surgeon is telling you something will hurt or take longer than it should to heal, make sure you do your research on the procedure and that particular doctor!

“I think these cellulite procedures work less effectively than advertised. The procedures that are completely non-invasive are a waste of money as far as I am concerned. Whether the procedure is heating or cooling the fat/cellulite, it is a waste.” There is VERY little evidence to show that non-invasive cellulite procedures work for permanent results. Often, results only last for a matter of hours due to the cellulite tightening, not actually going away.

In Your 50s and 60s:

  • You may be experiencing some saggy skin, most notably in the neck and facial area. Fillers, injectables and other non-invasive treatments are not the way to go, though. Surgically removing it (with a facelift) is the only option.
  • Facelifts are said to make you look 10 years younger. According to NewBeauty, it helps to restore the natural curvature of the face, while also making it appear firmer. It comes at a hefty cost —procedures range from $4000-$25,000, last 3-6 hours and require 10-14 days of recovery.
  • Loose skin in the neck area can be easily removed with a neck lift. It can also be pretty pricey — between $3500 – $10,000 for 1-3 hour procedure with 7-14 days of recovery.

Dr. Wigoda says: “Patients in this age range definitely are candidates for fillers and Botox. However, these don’t tighten the neck or face skin. They can camouflage areas that are aging so they look more youthful.” One of the most important parts of plastic surgery is to make sure you are informed of what exactly is happening/being done to your body. According to the American Society of Plastic Surgeons, the number of annual Botox injections has increased 748% from 2000 to 2014. This leap in Botox use can be attributed to an increase in education about the products used for fillers, which have been proven safe to use and even have excellent side effects, like reducing migraines. However, being educated about the product doesn’t mean much if you’re not researching the person performing the procedure.

When it comes to cosmetic procedures, Botox ranks number one in the country, which is great because it gives clients a non-invasive option with minimal recovery time. However, the reasons clients love Botox are the same reasons trendy medical spas like them. The multi-billion dollar Botox industry can be done by estheticians out of medical spas or even dentist offices. There are over a dozen product varieties, each with a specific purpose and focal area so make sure you are getting the right one for the best results.

  1. If something goes wrong, what happens next?

When done incorrectly, negative reactions can occur! Can your procedure be reversible? If so, how long will it take? Botox can last for months while some fillers can be immediately dissolved.
